Give the learner an opportunity to test his or her mastery of the concepts taught in this lesson. The test can be as simple as a multiple-choice quiz or as complex as actually performing a piece of work.
You can use some of the forms ingredients (radio buttons, check boxes, fill-in slots) to construct a sophisiticated test. You can then have an Evaluate button to submit the results to a script which would score them and trigger the appropriate feedback.
This template, just includes a multiple-choice exam. Note that each answer takes the user to a feedback topic. In this case, the fourth answer is the correct one. If you are using multiple-choice questions, be sure to vary which answer is the right one.
